Possible issues:
1. You are using autofocus and camera fails to focus?
2. Time value is too short for exposure and clearing of buffer?
3. Unreliable shutter release cable wiring?
What version of Papywizard are you using?
What camera and lens?

if AF is on it may happen that the cam can't focus and don't release

Not that the 2.0.0 version has some bugs (I will release a 2.0.1 version soon). But I don't think it is the problem here.
Try to increase the 'pulse width (high/low)' param in the Camera tab...

Not necessarilly the issue, but a few of us merlin/papy users have had issues with less than great connections with the shutter cable.
When the connection is suspect I had several shots in a very long sequence fail, the cable was the issue, as the cable that I modified had a poor camera connection.
Check and check again... IF it starts working again wiggle the plug at the camera and see if it returns... If that doesnt change, check the cable at the merlin side of the shutter cable, and unplug/replug...
Maybe something else, but for me and a few others it was simple cabling...
